Upfront Assessment of Need instructions and templates
Providers are responsible for conducting an Upfront Assessment of Need (UAN) with all prospective participants of Qualifications Projects. The assessment must take place before they can participate in a project.

Employment Services Referral Form
The UAN process includes completion of an Employment Services Referral Form if your student is registered with an Employment Service Provider.

Learner Support Services
Learner Support Services (LSS) provides individual support to students with barriers to training through case management.
Students are eligible for LSS if they are studying a qualification on the Subsidised Training List (including trainees and apprentices) and have complex support needs which interfere with their studies.
LSS Post Course Transition Support is also available to students who participate in a Qualification Project.
Students enrolled at school are not eligible for LSS.

Participant Commencement Form
Students need to complete and sign the Participant Commencement Form before they commence project activities.
Information collected through the Participant Commencement Form is entered by the provider into the Skills and Employment Portal. For further information, see Skills and Employment Portal Reporting Instructions.
The completed consent declaration pages for the ‘Collection and use of personal information’ must be electronically scanned and submitted to DIS.firstname.lastname@example.org within the timeframe specified in the Employment Projects Service Agreement (EPSA).
